- Custom param decorators extract from the request — they never authenticate, authorize, or run business logic. Auth lives in guards. - Default to `Scope.DEFAULT` (singleton). Use `Scope.REQUEST` only when state must be per-request and an `AsyncLocalStorage`-based context wouldn't do.
